Hi anon,
Gender realignment procedures are a process. Part of being able to undergo the surgeries a person wants to generally involve speaking with a mental health professional for a while beforehand. This is to help make sure that someone is mentally and emotionally prepared.
The counselor can also help with talking to the people in your life about how to come out or how to discuss the fact that you want to transition. If you are out as transgender to your partner, then talking with her about wanting to start the transitioning process may be easier than if she doesn’t not know you are trans.
I would suggest speaking to a trusted adult if you can and have them help you to find a counselor who specializes in working with transgender teens and transitioning. You can find a listing of trained sexuality counselors and therapists here.
